Summary Job Description: Physical Therapist Full Time, First Shift The Physical Therapist I shall evaluate and treat patients; develop a treatment plan; and set goals for patients. Patient and family education is included in the treatment plan and goals. Job Duties: - Completes appropriate functional evaluation including assessment of pain for various musculoskeletal/neurological conditions. - Plans treatment appropriately including input from patient and/or caregivers. - Provides services to improve functional independence and/or reduce pain such as: therapeutic procedures/exercises or activities to promote ROM, strength, coordination, balance, functional mobility (bed mobility, transfers, floor transfers), ADL training, gait training, balance and vestibular training, education, and use of assistive devices. - Receives and implements therapy orders. - Completes evaluation form, treatment notes, progress reports and D/C summaries in a timely manner and appropriately submits to referring provider.
